How to Become a Patient at Dodd Hall

If you or a family member is interested in being admitted to Dodd Hall Inpatient Rehabilitation, you must have a physician referral. You can ask the physician currently overseeing the care of the individual to contact Dodd Hall Admissions Office at 614-293-3824. If you are currently a patient at The Ohio State University Medical Center, please note that Dodd Hall is a separate admission process.

Steps to Admission at Dodd Hall include:

Referral Form Submission

  • Patients currently at The Ohio State University Medical Center:
The patient's physician is required to complete and submit the consult form (MC040178) in eResults

  • Prospective patients (not currently at The Ohio State University Medical Center): You or your physician should complete the patient referral form (must have physician sign it), and submit to the Admissions Office at Dodd Hall via fax (614-293-3198) or mail (Admissions, 480 Medical Center Drive, Columbus, OH 43210).

The Dodd Hall Admissions office will coordinate patient transportation with the staff of the patient's current facility. We will work together to establish an expected time of arrival. Arrival prior to 4 p.m. is requested in order to allow the medical team to do a thorough patient evaluation.

The length of stay at Dodd Hall is from five-20 days. Dodd Hall has both semiprivate and private rooms. We try to accommodate patient requests for private rooms, with prioritization given to patients who must have isolation due to special infection protection needs.
