Instructions for Requesting Medical Records

Release of information/correspondence is located in 140 Doan Hall and is staffed from 8 a.m.-4:30 p.m., Monday through Friday. An authorization form for the release of medical information must be completed for all requests. The authorization form should be mailed to the Medical Information Management Department. There are also authorization forms available in Spanish, Somali and Russian upon request. Please see additional information below.

For University Hospital, the Arthur G. James Cancer Hospital and Richard J. Solove Research Institute, The Richard M. Ross Heart Hospital and OSU Harding Hospital, fill out the authorization form and send it to:
