Hi! Giving our patients the opportunity to participate in their own care is a core element of P4 Medicine. One way we accomplish this goal is by making sure our patients and their families have the information they need to better understand their conditions and treatments. Our Patient Education team has worked hard to make sure our materials meet the most recent Joint Commission standards for patient communication, culture and health literacy. These materials are written in an easy-to-understand way that encourages patients to take an active role in their care.

Now, you can find these materials on a new and improved patient education website. The new site – available to staff and patients – provides high-quality, up-to-date, patient-friendly educational materials.

More than 5,000 documents are housed on the site, and many are available in up to 18 different languages. This site receives more than 50,000 staff visits a month through OneSource, and more than 70,000 visits a month through our public websites. That’s nearly one-and-a-half-million visits a year!

Not only are health systems across the United States using our patient education materials, they were also used by the World Health Organization during earthquakes in China, and by military facilities in Germany and during the Haiti earthquake last year. We’re building an international reputation!

The site also features an improved search function and an easier-to-use checkout cart. Clinicians will be able to pull education resources at the patient’s bedside and document that information into the medical record using IHIS. This will help improve documentation, workflow, patient safety and quality of care.

I’d like to thank all those who worked so hard over the last nine months to get this site launched, including our Patient Education team, Information Technology, and Communications and Marketing. You’ve given us a great example of what we can accomplish when we work together.
